What is the HOMA-IR formula?

HOMA-IR = fasting insulin (µIU/mL) × fasting glucose (mg/dL) ÷ 405, from Matthews et al. 1985. In SI units it's insulin (mU/L) × glucose (mmol/L) ÷ 22.5. This calculator handles the unit conversions for you.

What HOMA-IR is considered insulin resistant?

There's no single universal cutoff. 2.0 is a common watch line and about 2.5 is the usual research threshold (NHANES); under 1.0 reflects excellent sensitivity. Asian-population cutoffs run lower. Because insulin assays vary, track your trend on one lab rather than over-reading a single value.

Why measure insulin instead of just glucose?

Insulin rises to hold glucose normal for years before glucose itself moves. A fasting glucose of 95 mg/dL can look fine while a high insulin pushes HOMA-IR well into the resistant range. The resistance is only visible because you measured insulin.
